Parental Consent

Parental Consent for Minor Passport Applications (Below 16 Years)

Parental consent is required to issue a separate passport to a minor applicant under 16 years of age, when the mother, father, both parents, or legal guardian

resides in the UK or Ireland, and the child is living in Sri Lanka or another country.

Required Documents

1.     A duly completed Parental Consent Form

2.     Child’s Birth Certificate along with a photocopy

3.     Marriage Certificate (if available), along with a photocopy

4.     Applicant’s current passport and valid visa (e-visa share Code or Irish Residence Permit Card)

5.     Documentary evidence of legal guardianship or custody of the minor (if applicable)

Important:

·        The applicant must present the Consular Section in person.

·        If the parent or legal guardian is not a Sri Lankan citizen, holds refugee status, or is residing in Ireland and unable to attend the Consular Section in person,

         the Parental Consent Form must be signed in the presence of a Solicitor. The document must then be legalised by the Foreign, Commonwealth and

        Development Office (FCDO) in the UK or the Department of Foreign Affairs in Ireland, as applicable, before submission to the High Commission.

  • Please note that personal cheques will not be accepted.
  • Fees paid will not be refunded under any circumstances.
