Alteration of Passports

Alteration of the name of a female consequent to marriage → Form “O” (IM 37)

Applicants are required to submit the following documents

1.       Current passport of the applicant

2.       Original Birth Certificate along with a copy

3.       Original Marriage Certificate along with a copy

4.       Husband’s Passport along with a copy

5.       Valid UK or Republic of Ireland visa (E-visa share code or Residence permit card)
