Emergency Travel Document

 An Emergency Travel Document valid for six months may be issued to enable a Sri Lankan national only to return to Sri Lanka in case of an emergency in the following circumstances:    

 1.     The original passport is lost/damaged/stolen

 2.     The original passport has expired

 3.     The applicant does not have a Passport or a Travel Document and wishes to return to Sri Lanka

 4.     The applicant has withdrawn their appeal to extend a visa to stay in the UK/Republic of Ireland, or the relevant authorities have rejected their visa application

Required Documents for Emergency Travel Document (ETD)

Originals, along with one set of copy on a single-sided A4 paper, are required for the following supportive documents.

1.     Duly completed Form K 35 A Detailed instructions and forms are available at: https://hc-london.cwp-staging.econsulate.net/en/downloads

2.     A letter requesting an Emergency Travel Document

3.     Proof document for the reason of the emergency

4.     (03) identical colour photographs, Size: 3.5 cm x 4.5 cm (Link for Photo PDF) Sinhala / Tamil / English

5.     Expired Sri Lankan Passport (with photocopies of the data/photo page and observation/alteration pages-2,3,4 & 5)

6.     Police report OR Form D, Complaint Form & Statutory Declaration (click here to download) and previous passport copy (if passport is lost)-this form and declaration may be certified from the Consular section

7.     Birth Certificate

        o    Original or certified copy

        o    English translations alone are not accepted as originals

        o    In exceptional cases where the applicant’s birth certificate is unavailable, a search result certificate and the applicant’s child's birth certificate may be submitted.

8.     Proof of Legal Status in the UK or Ireland-One of the following must be provided:

        o    Valid visa [E-visa share code (starting from “S”) / Residence Permit Card for applicants in the Republic of Ireland], or/and

        o    UK/Ireland/Foreign Passport (for dual citizens)

9.     Affidavit (if applicable) Applicants holding Indefinite Leave to Remain (ILR), Permanent Residence (PR), or Settlement status for one year or more must submit an affidavit* confirming hey have not obtained British citizenship. This affidavit must be certified by the Consular Division. Certification fee: £57/-

10.    Sri Lankan National Identity Card (NIC) – (If available)

11.    Affidavit for applicants born outside Sri Lanka and aged over 21 years (Applicants must submit an affidavit  confirming their intention to retain Sri Lankan citizenship.)

12.    Travel Itinerary/Air ticket (14 days ahead).

Proof of Sri Lankan Citizenship

The following documents must be provided, if applicable:

• Dual Citizenship Certificate
• Certificate of Registration of Birth outside Sri Lanka (under section 5(2) of the Citizenship Act)
• Citizenship Registration Certificate (under Sections 8, 11, 12, or 13 of the Citizenship Act)
• Certificate of Grant of Citizenship (Indian Origin persons)
• Certificate of Grant of Citizenship (Chinese Origin persons)

Additional Requirements (If Applicable)

Inclusion or Change of Husband’s Name

Married female applicants wishing to include or change their surname in the passport must submit:

• Original Marriage Certificate (Translations are not accepted as originals)

Minor Applicants (Under 16 Years of Age)

The following documents must be submitted (originals with photocopies):

6.     Child’s Birth Certificate or Consular Birth Certificate

7.     Citizenship Certificate or Citizenship payment receipt (if born outside Sri Lanka, and applying for the first time, the payment receipt can be submitted)

8.     Consent from both parents (click here to download the format)

9.     Parents’ valid passports (with photocopies of data and observation pages/if parents have an invalid passport, the parent’s passport renewal payment receipt must be submitted.)

10.   Parents’ valid visas [E-visa share code (starting from “S”) / Residence Permit Card for applicants in the Republic of Ireland],

11.   Parents’ Marriage Certificate (if married)

12.   Additional documents if applicable:

         o    Court order (if parents are divorced)

         o    Death certificate (if parent is deceased)

         o    Legal guardianship documents

13.   Travel Itinerary/Air ticket (14 days ahead).

Important:

The child and both parents must be present when submitting the application.

ETD Applications for Buddhist Priests

Applicants must submit:

• Samanera Certificate, or Higher Ordination Certificate

Prescribed Fees and Payment Details

• Method of Payment – Cash, Debit/Credit card,
• Please note that personal cheques will not be accepted.
• An additional amount will be charged if photocopies are made at the High Commission. 
• Fees paid will not be refunded under any circumstances.

ETD Collection by Post

·        If the applicant is unable to collect the ETD in person due to exceptional circumstances, the ETD may be returned by post. The applicant must send:

         o    A written request

         o    The previous passport

         o    The original yellow payment receipt

         o    A self-addressed prepaid Special Delivery envelope

·        Applicants residing in the Republic of Ireland must arrange ETD collection through a courier service. Before arranging collection, applicants must send their

         previous passport and payment receipt to the High Commission. Courier collection details must be emailed in advance.

·        The High Commission is not responsible for any passports or documents lost or damaged sent by post
