Sri Lankan Driving Licence Services
Renewal of Sri Lankan Driving Licence
Documents Required
- Duly completed MTA 30/2 offline application form (original) (Available at the High Commission. Photocopies cannot be used. Form must be completed in BLACK ink only.)
- Current Driving Licence issued by the Department of Motor Traffic, Sri Lanka
- Medical Certificate from a GP confirming the applicant is fit to drive (click here to download the format)
- Original valid passport with a copy
- Valid visa (e-visa share code or Irish Resident Permit card with a copy)
- Original National Identity Card (NIC) with a copy (Not mandatory if the NIC number appears in the passport or Driving Licence)
- Two black and white passport-size photographs (35mm × 45mm)
Apply for a Duplicate Sri Lankan Driving Licence
A duplicate licence may be issued if the original licence is lost, damaged, or illegible.
Documents Required
- Duly completed MTA 30/2 offline application form (original) (Available at the High Commission. Photocopies cannot be used. Form must be completed in BLACK ink only.)
- Police complaint report from the local police station where the applicant resides for the lost driving licence
- Medical Certificate from a GP confirming the applicant is fit to drive (click here to download the format)
- Original valid passport with a copy
- Valid visa (e-visa share code or Irish Resident Permit card with a copy)
- Original National Identity Card (NIC) with a copy (Not mandatory if the NIC number appears in the passport or Driving Licence)
- Driving Licence Number or NIC Number must be provided to trace the records (Driving Licence copy can be submitted if available)
- Two black and white passport-size photographs (35mm × 45mm)
Change of Personal Details on Sri Lankan Driving Licence
Applicants may request changes to name or address details.
Documents Required
· Duly completed MTA 30/2 offline application form (original) (Available at the High Commission. Photocopies cannot be used. Form must be completed in BLACK ink only.)
· Current Driving Licence issued by the Department of Motor Traffic, Sri Lanka
· Original valid passport with a copy
· Valid visa (e-visa share code or Irish Resident Permit card with a copy)
· Original National Identity Card (NIC) with a copy. (Not mandatory if the NIC number appears in the passport or Driving Licence)
· Supporting documents relating to the requested change, such as: Birth Certificate/Marriage Certificate/Divorce Certificate/Resident Certificate (for address changes)
· Two black and white passport-size photographs (35mm × 45mm)
Instructions for Completing the Application
- MTA 30/2 offline application forms are available at the Sri Lanka High Commission in London, and cannot be used with photocopies
- Applicant should come in person to submit their application.
- The form must be completed in English BLOCK LETTERS using BLACK ink only.
- Applicants must write the Sri Lankan Address and the National Identity Card (NIC) number in the relevant cages in the application.
- Do not write outside the scanning area of the form.
- In Part C of the application form, details of the Medical Certificate must be included.
- The medical certificate must indicate the applicant’s blood group.
- Black and white photographs must have a white background. (Passport Size: 35mm × 45mm)
